Just the domain is fine, no need for https://. This is the exact origin the badge will be embedded on, and a badge issued to one domain shows a warning on any other.
If there is one. Leaving it blank is fine and is not held against you. It means the report will say the code was not reviewed, on the badge as well as the page.
Admin keys, upgrade paths, anything unusual. Telling us up front is not held against you. Finding it ourselves after you did not mention it is.

What happens next

  1. We connect a wallet to your site and watch what it asks for.
  2. We work through every transaction and signature the site can produce, and compare what it shows against what it actually submits.
  3. We read the contract, if the source is public or you send it.
  4. You get the findings. If something needs fixing, you fix it and we look again.
  5. If it passes, you get a badge and a dated public report page listing exactly what was examined.

Things worth knowing first

  • A review that does not pass stays private. Fix it and re-submit at no extra cost.
  • A badge that is issued and later revoked is published, with the reason. That is the whole point of the badge meaning anything.
  • A review is dated and expires. It describes your site on one day, and the report says so plainly.
  • We do not review the price, the tokenomics, or whether your idea works.
  • We cannot tell anybody what you will do next, and the report says so.
